I acknowledge that the coaching services provided by Dawn Ezelle through Inspiring Today Ministries are faith-based and designed for educational, spiritual, and personal development purposes only. These services are not intended to diagnose, treat, cure, or prevent any disease, mental health condition, or physical illness.

I understand that:

• Coaching is not a substitute for professional medical advice, diagnosis, or treatment.
• No licensed occupational therapy or psychological counseling is being provided.
• I am solely responsible for my own decisions, actions, and results from the coaching process.

If I have a medical or psychological condition that requires licensed care, I agree to seek appropriate assistance from a licensed physician, therapist, or healthcare provider.

By signing this waiver, I release and hold harmless Dawn Ezelle and Inspiring Today Ministries from any and all liability, damages, or claims arising out of or in connection with my participation in coaching sessions. I agree that I am voluntarily participating in coaching services with full knowledge of the nature of the services provided.

If the client is under the age of 18, a parent or legal guardian must sign the following:

I, the undersigned, am the parent or legal guardian of the minor named above. I give consent for my child to receive faith-based coaching services from Dawn Ezelle through Inspiring Today Ministries. I understand and agree to the nature of the services as described in this form and release Dawn Ezelle and Inspiring Today Ministries from all liability as outlined above.

This coaching agreement outlines the expectations and responsibilities between Dawn Ezelle (Coach) and the undersigned (Client). Coaching is a faith-based, forward-focused process that supports spiritual growth, life balance, and emotional wellness. It is not therapy or medical treatment.

• Sessions will last 45–60 minutes.
• Coaching will take place via Zoom, phone call, or in-person (as mutually agreed).
• The number and frequency of sessions will be determined based on the client’s goals.

• All coaching conversations are kept strictly confidential to the extent permitted by law. Information will not be shared without written consent, unless required by law or in cases of danger to self or others.

• Please provide at least 24 hours’ notice for cancellations or rescheduling.
• Missed appointments or late cancellations may result in the forfeiture of that session.
• Emergency exceptions may be made at the coach’s discretion.

Inspiring Today Ministries offers a dual structure to make coaching accessible to all:

Option A: Coaching Packages (Set Fee)

Option B: Donation-Based Coaching

• For individuals in ministry, transitional seasons, or financial hardship, coaching is available by donation.

  Inspiring Today Ministries is a registered 501(c)(3) nonprofit. Donations are tax deductible.

By entering this coaching relationship, you agree to:

• Show up fully present and open to growth
• Take personal responsibility for applying what is learned
• Communicate honestly about progress, feedback, or concerns

The coach (Dawn Ezelle) is not providing occupational therapy or medical treatment. Coaching is not a substitute for licensed mental health or medical care. If you have concerns outside the scope of coaching, you are encouraged to seek professional help from a physician or therapist.
